Vagal AF is more frequently seen in younger patients (30-50 years old), typically men and usually the heart is structurally normal on echocardiography. Commonly this type of AF stops in the morning or during periods of exercise and can be precipitated by cough, nausea, after eating, swallowing and ingestion of cold foods and drinks. When to suspect vagal AF: If the arrhythmia occurs at rest, after meals or during sleep then it is more likely to be vagal stimulated. So called vagal AF is enigmatic but should be more recognised. This is important because it needs to be managed differently from other more common types of atrial fibrillation. For some patients increased vagal activity is associated with the initiation of atrial fibrillation. It is part of the autonomic nervous system and its cardiac action is to slow the heart. The vagus nerve wanders from the brainstem throughout the chest and abdomen innervating the heart, lungs and intestines. The initiator of the AF episodes can often be overlooked in the consultation but is very important particularly as vagal AF will gets worse when treated with beta blockers. That was a tipping point in my mind where I realized something was wrong. The client was ecstatic and happy that I had finished so quickly, and we both parted ways after the project. I had just created a high quality website for this individual, and I had only been paid $180 for it. I finished the website in 3 hours…Īnd worst of all, the website looked freaking good. He gave everything to me and then I plugged it into a new website design. He provided all the assets, the scope of work, the site map and the content upfront. I was happy as it looked like a great client. An example of hourly pricing at its worstĪ client came to me via Upwork at my $60 per hour rate. Clients care that the work is done and it is done well. ![]() This all culminated in one specific project that completely changed my mindset on pricing.Ĭlients don’t care if it takes you 20 minutes or 20 hours to complete the project.
